What is Parinauds Syndrome ?  

Parinaud’s syndrome refers to the group of abnormalities of eye movement and pupil dysfunction. There is an inability to move the eyes up or down due to compression of the vertical gaze center at the rostral insterstitial nucleus of medial longitudinal fasciculus.


Cause(s) :  
  • Injury to the dorsal midbrain
  • Loss of blood supply resulting in damage to the mesencephalic tectum
  • Brain tumours in the pineal gland
  • Multiple sclerosis in women in 20s-30s
  • Stroke if the upper brainstem in older patients
Symptoms :  
  • Paralysis of upgaze (downward gaze may or may not be affected)
  • Pupils become mid-dilated and show light-near dissociation
  • Attempts at upward gaze often produce nystagmus
  • Eyelid retraction
  • Bilateral papilledema
Diagnosis and Tests :  
  • Neurological tests of the eyes
