Overview of Vestronidase Alfa

• Vestronidase alfa injection for intravenous use has been approved by US FDA in November 2017 for the treatment of sly syndrome.

Why is Vestronidase Alfa Prescribed? (Indications)

Vestronidase alfa injection is a recombinant form of a human lysosomal enzyme called beta-glucuronidase which is prescribed to treat a genetic or a metabolic disorder known as mucopolysaccharidosis VII or Sly syndrome in which the individual lacks beta-glucuronidase.

Vestronidase alfa does not provide a cure to Sly syndrome but helps by improving the walking ability in people with this condition.

When should Vestronidase Alfa not be taken? (Contraindications)

Vestronidase injection should not be used in patients who are allergic to vestronidase alfa.

What is the dosage of Vestronidase Alfa?

• The recommended dose of vestronidase alfa is 4 mg/kg body weight once for every two weeks which should be administered over a period of 4 hours.

• Patients should be pre-medicated with non-sedating antihistamine either with or without fever medicine (Paracetamol) 30 to 60 minutes before starting the infusion.

How should Vestronidase Alfa be taken?

• Vestronidase alfa comes as an injection which has to be given by a healthcare person directly into the vein.
• It has to be diluted with 0.9% sodium chloride with the final concentration of 1:1 before infusion.
• Before dilution, the vial should be inspected for the presence of any particulate matter or discoloration and should be discarded if present.
• During the first 1 hour duration, 2.5% of the total volume of the infusion should be administered, and the remaining can be infused over the next 3 hours.

What are the warnings and precautions for Vestronidase Alfa?

• Patients should be monitored closely during and for up to 1 hour after administration. If any systemic reactions occur, the patient should be treated with appropriate medical care.

• Use the diluted solution immediately. If using immediately is impossible, store in a refrigerator at 2°C to 8°C for up to 36 hours and keep at room temperature for about 6 hours to make the infusion to come at room temperature before administration.

• If the diluted solution is not used up in 42 hours, discard the solution.

Common: Swelling of hands or legs, diarrhea, rashes, pain or swelling or irritation at the injection site
Others: Development of anti-drug and neutralizing antibodies, febrile convulsion, itching, anaphylaxis
