Treatment of Premature Ejaculation

Premature ejaculation is treated with counseling, various exercises, positions and maneuvers, and medications.


An occasional instance of premature ejaculation is not cause for worry, but if it affects the relationship between the partners it has to be tackled.

The first step in the treatment of this sex problem is for the couple to understand that there is nothing seriously wrong with the male and reassurance that his performance can be enhanced and made satisfying to both partners.

A counseling session with both the partners can help in better understanding of the problem. Emphasizing the importance of relaxation techniques, foreplay, well lubricated vagina before penetration and letting go the first time but trying to delay the second time may help the couple overcome the problem without the need for medications.

Some of the techniques used to delay ejaculation are:

Squeeze Technique

When the man realizes he is going to ejaculate, his partner squeezes the glans penis (i.e. the head of the penis). This helps to delay the ejaculation process. Once a couple is familiar with this technique it enables them to enjoy a longer a longer period of erection before ejaculation.

Condoms

Condoms may be used to decrease the sensitivity of the penis.

Masturbate before sex

The male partner is asked to masturbate about two hours prior to intercourse, so that a second erection is delayed when he actually has sex with his partner.

Kegel's Exercises

These exercises are recommended by some people to strengthen the muscles of thepelvic floor, so that the ejaculation is delayed. This exercise is performed as follows- the muscles of the pelvic floor are tightened and held in the taut position for at least 10 seconds. The exercise is repeated about 30 times everyday. This is the same muscle one would tighten to stop the flow of urine midway after beginning to urinate.

Woman on Top sexual position

"Woman on top" is a popular position and it is claimed to be helpful in delaying the ejaculation. An understanding partner has better control of the male organ in this position and can perform the act less vigorously and this results in delay in ejaculation.


Medications Used to Treat Premature Ejaculation Include:

Premature ejaculation or early ejaculation, a common sexual problem faced by men can be treated by using the following medications:

Selective Serotonin Reuptake Inhibitors (SSRIs) - Drugs like paroxetine, fluoxetine and sertraline act by inhibiting sexual arousal and help prevent premature ejaculation.

Tricyclic Antidepressants - Clomipramine is taken several hours before sexual intercourse to avoid early ejaculation. However usage of clomipramine may sometimes result in unwanted side effects which include fatigue, dizziness and reduced blood pressure.


Topical Anaesthetics - Drugs that cause numbness and relieve pain in a particular area without loss of consciousness like benzocaine, prilocaine, lidocaine are widely available in the form of creams and sprays. They are applied topically 15 minutes before intercourse to desensitize the penis thereby avoiding early ejaculation.

Phosphodiesterase Type 5 (PDE-5) Inhibitors - Phosphodiesterase 5 inhibitor drugs like Sildenafil, Vardenafil and Tadalafil can be used to treat erectile dysfunction or impotence. Erectile dysfunction is the inability to achieve or sustain an erection during sexual activity

Tramadol, a centrally acting opioid analgesic was found to show progressive sign in the treatment of premature ejaculation, reveal research studies.
