"The agreement combines the extensive resources of Centura Health, Primary Physician Partners, Physician Health Partners (PHP) and Anthem and is based on a mutual goal to innovate within the Medicaid delivery system," says PHP's Ken Nielsen, representative on CCHA's board of directors. "By breaking down the typically siloed nature of health care, we aim to make the best use of limited resources to ensure members are receiving the right care, at the right time and in the right setting to eliminate duplication and over-utilization."

The collaboration pairs the pioneering success of CCHA's primary care, provider-led delivery model with Anthem's robust experience as an accredited NCQA Managed Behavioral Healthcare Organization. Through this partnership, CCHA and Anthem combine forces as a united entity to provide a unique and influential blend of primary care medical providers, specialists, hospitals and behavioral health services under one company.

"This new entity will support the delivery of seamless, integrated care to decrease fragmentation, improve service coordination and meet the member's identified health goals," says Anthem's Manny Arisso, JD, LHRM, behavioral health expert on CCHA's board of directors. "We are excited to leverage our experience and believe this partnership will uniquely position CCHA to support the delivery of integrated, holistic care to some of Colorado's most vulnerable populations."

About Colorado Community Health Alliance: CCHA was founded in 2010 to specifically meet the needs of Colorado Medicaid members by bringing together the local expertise of Centura Health (the largest health system and specialty provider network in Colorado), Physician Health Partners (a local management services organization known for their care coordination and practice quality improvement programs) and Primary Physician Partners (a large group of local primary care providers). As a peak performing Regional Care Collaborative Organization, CCHA has proudly served the Colorado Medicaid population with a history of strong performance outcomes, a large and established provider network, dedication to reinvestment in the community and collaboration with the Colorado Department of Health Care Policy and Financing.
