Advancements in follicular unit extraction (FUE) techniques make it possible for patients to avoid the linear scar that is the hallmark of traditional hair transplantation surgery. This allows patients who undergo the procedure to maintain a short hairstyle without visible scarring. Regardless of the donor technique used, patients still must be aware of the possible limitations of the donor hair quality. Depending on the size and density of the beard the patient is expecting, a secondary procedure may be required.

The donor area is typically shaved, although a no-shave alternative may be offered for some patients. The donor area is prepped for the procedure and local anesthesia is applied to the scalp. Using a tiny drill, individual follicle extractions are made in the donor area and distributed evenly throughout the scalp to avoid bald spots.

Once the extractions have been made, local anesthesia is applied to the face where the recipient sites will be made. Tiny slits are made where the hair will be grafted. The key, says Dr. Bared, is to make the incisions at an ultra-acute angle determined by the existing hairs on the face. Typically growth is downward but can be more anterior closer to the mouth area, or slightly laterally on the mustache. If it is determined that more grafts are needed, they are extracted from the scalp at this time.

There is a high success rate for hair transplants from the scalp to the face. However, complications can occur. Poor angulation can give the beard an unnatural appearance. This is more likely in the mustache area than in other parts of the face. According to Dr. Bared, the best way to avoid improper angulation is to use the smallest incision possible at an acute angle.
