VNA Philadelphia Partners with HRS to Reduce ED Visits and Lower Hospital Readmissions

PHILADELPHIA, Sept. 5, 2018 /PRNewswire-PRWeb/ -- VNA Philadelphia, the oldest and largest non-profit home health and hospice provider in the Philadelphia area, has partnered with Health Recovery Solutions (HRS) to reduce hospital readmission rates and limit avoidable emergency department visits for their patients. VNA Philadelphia has established a comprehensive telehealth program with the HRS software, known as HELM – Health Education and Lifestyle Management.

The HELM program will serve medically complex patients who are at risk for hospital readmissions. VNA Philadelphia also plans to provide telehealth interventions to patients with chronic illnesses, such as CHF, COPD, and diabetes, to promote independent symptom management and improve patient quality of life. As the program develops, VNA Philadelphia will expand HELM to any patient who can benefit from telehealth technology.

Patients participating in HELM are provided with 4G tablets pre-loaded with the HRS software. The tablets pair with Bluetooth peripheral devices that allow patients to track relevant physiological data, such as blood pressure, weight, heart rate, and glucose levels. The patient information automatically transmits to the tablet and is electronically delivered to the patient's nurse, allowing clinical staff to address changes in medical status in a timely manner.

The HRS software also offers video conferencing and wound imaging features, which enhance patient communication and diagnostic accuracy. VNA Philadelphia is particularly excited about these features, as they will allow the clinicians to deliver more targeted treatment interventions and management guidelines to their patients. Additionally, the HRS software will improve the workflow at VNA Philadelphia because it integrates with Strategic Healthcare Programs (SHP), an application they currently use to monitor quality of care.
