Support professionals may review team plan minutes and create an ISP Agenda to drive meetings based on the needs and areas identified through the individual's Personal Focus Worksheet. Interconnected modules ensure that service data collected in Therap generates reports and drives the progress of outcomes.
Additionally, the Medication History tool allows RNs and LPNs to keep record of medications. Therap's Medication History includes detailed information on side effects, drug-allergy reactions and drug-drug interactions. With the medication history tool, nurses have the necessary information to avoid or minimize medication errors.
