Community-based lifestyle intervention aims to reduce risk, improve outcomes

PHOENIX, Dec. 6, 2018 /PRNewswire/ -- Solera Health, a leading integrated community network, today welcomed Bridgeport, CT-based Monitor My Health, Inc. into its national network of Centers for Disease Control and Prevention (CDC)-recognized diabetes prevention program (DPP) providers.

In a diabetes prevention program, participants work in groups with a trained lifestyle coach to learn the skills needed to lose weight, be more physically active, and manage stress.  For people with pre-diabetes, losing five to seven percent of body weight can cut the risk of developing type 2 diabetes in half.  A New England Journal of Medicine study showed that lifestyle intervention reduced the incidence of type 2 diabetes significantly more than an anti-diabetic medication.

Monitor My Health provides this lifestyle intervention via daily interactive education and activities that focus on diabetes and heart disease prevention while helping participants appreciate the costs, complications, and effect of the disease on caregivers and family members. Their flexible, traveling team teaches people how to make lifestyle changes that reduce risks and improve health outcomes from the convenience and comfort of a residential, workplace, church, senior center, or other community setting.

"Solera and Monitor My Health share the common mission to prevent diabetes and heart disease by educating people on the value of screening and lifestyle changes," said Dr. Dana Wade, M.D., Monitor My Health's founder and CEO.  "Solera offers technology and administration that complements our services and matches us with DPP participants seeking the information and motivation needed to achieve their goals of better health, while allowing us to focus on program delivery and health outcomes."

"We are very pleased to offer Monitor My Health as an option to eligible participants in Connecticut as we continue to scale the diabetes prevention program nationwide," said Brenda Schmidt, Solera Health's CEO. "The Monitor My Health team has a real passion for empowering people with the knowledge that diabetes is preventable, motivating them to take the necessary steps to achieve their goal of better health."
