Community-based lifestyle intervention aims to reduce risk, improve outcomes

PHOENIX, Dec. 6, 2018 /PRNewswire/ -- Solera Health, a leading integrated community network, today welcomed Bridgeport,

CT-based Monitor My Health, Inc. into its national network of Centers for Disease Control and Prevention (CDC)-recognized diabetes prevention program (DPP) providers.

In a diabetes prevention program, participants work in groups with a trained lifestyle coach to learn the skills needed to lose weight, be more physically active, and manage stress.  For people with pre-diabetes, losing five to seven percent of body weight can cut the risk of developing type 2 diabetes in half.  A New England Journal of Medicine study showed that lifestyle intervention reduced the incidence of type 2 diabetes significantly more than an anti-diabetic medication.  

Monitor My Health provides this lifestyle intervention via daily interactive education and activities that focus on diabetes and heart disease prevention while helping participants appreciate the costs, complications, and effect of the disease on caregivers and family members.
