The dynamic, two-day event will convene more than 900 health care leaders from across the nation to discuss the biggest trends and innovations in population health. Nationally recognized subject matter experts will take the stage to share their vision, strategies and experiences in advanced payment models. The symposium will feature educational sessions, peer-to-peer networking opportunities and real-life case studies around the unique challenges organizations face in value-based payments.

The first day of the symposium kicks off with industry experts sharing their knowledge about changes affecting new payment models and how organizations can best position themselves to win at population health. Policy leaders will dive into new rules facing Medicare accountable care organization (ACO) participants and help participants understand how they can thrive under a risk-based model. The afternoon will feature specialized tracks that will address the real-world concerns of new and experienced ACOs, and Transforming Clinical Practice Initiative and CPC+ participants such as change management solutions, prevention and wellness initiatives, physician leadership, remote patient monitoring and more.
The second day will show organizations how they can drive clinical outcomes. Attendees will hear best practices from a panel of peers on how to successfully implement a Behavioral Health Integration program. Clinical leaders will dig deep into analytics to help participants gain actionable insight their community health and improve practice workflows. Afternoon breakout sessions will focus on leadership, educational and innovative topics such as optimizing Hierarchical Condition Categories coding, preparing for financial sustainability, diabetes care, managing socio-economic factors, care transitions and more.
