NORWELL, Mass., Feb. 26, 2019 /PRNewswire-PRWeb/ -- NVNA and Hospice has recently launched a telehealth program with Health Recovery Solutions (HRS) to enhance remote monitoring for patients with chronic illnesses, such as CHF, COPD, and Diabetes. NVNA and Hospice uses the HRS telehealth platform to keep patients safe and healthy at home, while improving their independence in disease management.

The team at NVNA and Hospice has utilized telehealth for many years, but recently decided to partner with HRS because of the platform's strong potential to reduce hospital readmissions and promote patient education. Patients placed on the telehealth program at NVNA and Hospice are provided with 4G tablets pre-loaded with the HRS software. The tablets are paired with disease specific Bluetooth biometric devices that allow patients to capture their blood pressure, weight, heart rate, and more. This information automatically transmits to the tablet and is electronically delivered to the patient's clinician.

NVNA and Hospice has a dedicated nurse who centrally monitors all telehealth patients. This organizational structure allows their clinical team to quickly address any abnormalities in a patient's status. Additionally, clinicians can keep in touch with patients through the tablet's text, call, and video chat features. The team at NVNA and Hospice is particularly excited about using video capabilities to conduct education sessions with patients to further facilitate patient engagement and participation in disease management.
