The level of outreach is structured according to the degree of the alert pre-selected by the care team, patient and caregiver. Based on the selected responses, a live outreach call may be made. This one-on-one attention supports and motivates the patient, resulting in healthier outcomes. Purely technology based organizations do not provide the wrap around services such as initial and ongoing medication reconciliation, troubleshooting technology related issues, triaging symptomatic calls and live education that is required to support medication adherence.
