Industry leaders to speak about disease management solution at Salesforce annual conference

SEATTLE, and SAN FRANCISCO, Sept. 20, 2018 /PRNewswire-PRWeb/ -- MCG Health, part

of the Hearst Health network and a leading provider of informed care strategies, and Cadalys, a software innovator with Salesforce® expertise, will present their co-developed solution, Cadalys Care (Informed by MCG), at Salesforce's annual Dreamforce event which takes place September 25-28, 2018 in San Francisco, California.

Cadalys Director of Product Development, Jon Tresko, and MCG Associate Director of Product Management, Marivie Lanter, RN, BSN, will present the breakout session, "Intelligent Care Management with Evidence-Based Guidelines," on Thursday, September 27 from 11:00 am to 11:20 am at the Moscone South, Partner Theater 1. This session will discuss how to continually optimize care planning and improve outcomes by leveraging evidence-based guidelines via Salesforce's workflow automation and analytics tools. Executives in healthcare and life science fields are strongly encouraged to attend.

In May 2018, MCG and Cadalys launched Cadalys Care (Informed by MCG) for Salesforce Health Cloud (available through the Salesforce AppExchange®). The solution makes evidence-based care guidance easily accessible and gives care providers a complete view of a patient's care journey, including current conditions and medications. Care team members can access daily task/patient prioritization dashboards, interact with other care managers via the Chatter tool, quickly identify patient progress using a timeline of events, and customize panels for high-level views of relevant information. Cadalys Care (Informed by MCG) also features a disease management component which includes assessments and care plans for chronic condition management and transitions of care, and tools to help clinicians determine appropriate levels and settings of care. In addition, Cadalys will also be premiering new versions of Cadalys Care three times per year (alongside major Salesforce releases) including tools for utilization management and triage chatbots.

The Dreamforce event is widely considered the largest single-vendor technology conference in the world. It brings together thousands of Salesforce users and partners to explore new ways to engage with its cloud-based CRM platform. Cadalys Care (Informed by MCG) will be featured in the Moscone South building (booth 1741) during the conference, and the main Salesforce Health Lodge booth will feature live demos of the Cadalys Care solution. Booth 1741 will also have additional information on integrations between MCG and Cadalys for post-acute and home care which deliver evidence-based guidelines and benchmarks to improve the patient journey outside hospital walls.
