HabitNu Partners with Solera Health to Offer In-Person DPP to Meet the Needs of Medicare Beneficiaries

PHOENIX, Nov. 20, 2018 /PRNewswire/ -- Chicago, IL-based HabitNu is expanding their digital lifestyle modification and diabetes

prevention program to include an in-person option delivered through a nationwide network of independent pharmacies, fitness facilities and local community organizations and joins Solera Health's network of Medicare Diabetes Prevention Program (MDPP)

To meet MDPP requirements, HabitNu has added recognized in-person coaching sessions delivered weekly at community locations in support of the diabetes prevention skills taught through the HabitNu application.

Based on the proven curriculum developed by the Centers for Disease Control and Prevention (CDC), HabitNu's diabetes prevention program (DPP) is designed to fit the busy lifestyle of participants. The program features a flexible, personalized, and culturally sensitive approach to motivating healthy behavior change through a combination of small cohorts built to nurture engagement and accountability, lifestyle coaching, phone-based monitoring, and free supportive tools.

The HabitNu program has generated excellent outcomes with a 98 percent retention rate, 66 percent of participants losing weight, and 69 percent of participants improving clinical values.
