Extended partnership empowers community with population health management infrastructure across CRISP Footprint.

today announced that it has extended its agreement with the Chesapeake Regional Information System for our Patients (CRISP), a non-profit regional health information exchange (HIE) supporting Maryland, the District of Columbia, and parts of Virginia and West Virginia. The agreement enables organizations across the region to access GSI Health's configurable GSIHealthCoordinator population health management platform to enable efficient and effective care management for value-based programs.

CRISP's partnership with GSI Health brings the benefits of integrated care coordination and analytics to every organization in CRISP's footprint as part of its HIE service, enabling the creation of interdisciplinary care teams and centralized care plans and population health management information to be incorporated into workflows at the point of care. The GSIHealthCoordinator platform supports care transformation organizations (CTOs) and others providing collaborative care to improve efficiency and quality measures and increase profitability through performance-based incentives.
