The Centers for Medicare and Medicaid Services (CMS) have stated that achieving health equity and driving improvements for all patients require further investment in tools that address social determinants and close care gaps. However, constraints on budgets and resourcing make it increasingly difficult to provide high-touch services to all patients, particularly those populations with unique or complex needs.

Docent Health uses its experience intelligence platform to efficiently and effectively personalize interactions for entire populations across phone, email, and text, but goes a step further by also providing patients access to a live, non-clinical navigator. Together, the technology and the "docent" individualize support and guidance across local community resources, educational programs, and care preparation, while also orchestrating activities with care teams and social support resources.

Jacqueline Thong, Docent Health's Senior Director of Product Performance, will be presenting the research and results at Activate 2019 in Los Angeles, CA on October 10th. A preview of some of the research findings are:

  • 89% of patients actively engaged with the Docent Health program when offered
  • 76% of all patient interactions are through text message
  • 3.5x docent navigator efficiencies gained, allowing navigators to spend more time with patients who need additional support
