Health plan will introduce innovative outreach model to BadgerCare Plus enrollees in six counties in Southeast Wisconsin

MADISON, Wis., April 14 /PRNewswire/ -- CommunityConnect HealthPlan announced today that the Wisconsin Department of Health Services intends to award CommunityConnect with the Medicaid Managed Care contract to provide benefits to enrollees in the BadgerCare Plus program in Milwaukee, Racine, Kenosha, Waukesha, Washington and Ozaukee Counties. CommunityConnect was one of four health plans selected to serve more than 240,000 enrollees in BadgerCare Plus. Beginning in September 2010,  BadgerCare Plus enrollees will have the option to choose CommunityConnect HealthPlan to provide their BadgerCare Plus benefits.

CommunityConnect benefits will be offered by Compcare Health Services Insurance Corporation, whose parent company's State Sponsored Business division has provided managed health care benefits in ten states to more than 1.7 million individuals who qualify for state funded programs including Medicaid, State Children's Health Insurance Program (SCHIP) and high risk pools.

We know that during this difficult economy, not only are families worried about accessing health care, but they may also need help with resources to address housing, job placement or other social services," Hayden said.

An integral part of CommunityConnect's care management plan will focus on addressing the critical health care needs of BadgerCare Plus enrollees to help ensure they are accessing needed health care services. Programs to improve outcomes for high-risk pregnant women, emergency department utilization and medical home programs that help members foster a relationship with a primary care physician are just some examples of the programs that will be available to CommunityConnect members.  

Group Health Cooperative (GHC) of Eau Claire also joins CommunityConnect's innovative team, providing more than 20 years of local expertise with the BadgerCare Plus program and critical programs such as claims processing, customer and provider service.

"In addition to extensive community outreach, our model is rooted in strengthening care coordination by working collaboratively with physicians so that together we can meet help meet the health care needs of our members," said Hayden. He added that that CommunityConnect will use state of the art health care information technology to share information with providers and members to better coordinate care and increase prevention. "We are confident that our innovative approach to Medicaid Managed Care will significantly help improve member health outcomes and improve their quality of life."
