Overview of Ibalizumab-uiyk

• Ibalizumab-uiyk injection is a non-immunosuppressive monoclonal antibody approved by FDA in March 2018 to treat adults with multidrug-resistant HIV-1 infection.

Why is Ibalizumab-uiyk Prescribed? (Indications)

Ibalizumab-uiyk intravenous injection is prescribed along with other antiretroviral medications to treat human immunodeficiency virus-1 (HIV-1) infection in adults. It is used for HIV patients with-
• Previous history of antiretroviral therapies
• Multidrug-resistant HIV-1 infection
• Failure or loss of response with other antiretroviral medicines

Ibalizumab-uiyk is a post attachment inhibitor that binds to CD4 glycoprotein, the primary receptor for human immunodeficiency virus and blocks the viral entry process into the immune cells.

When should Ibalizumab-uiyk not be taken? (Contraindications)

Ibalizumab-uiyk should not be used in patients who are-

• Allergic to ibalizumab-uiyk
• Pregnant and breastfeeding
• Below 18 years of age

What is the dosage of Ibalizumab-uiyk?

• The recommended dose of ibalizumab-uiyk is a single loading dose of 2000 mg initially followed by a maintenance dose of 800 mg administered for every two weeks.

• Dose modification is not necessary while treating with other antiretroviral medicines or while undergoing any other treatment.

How should Ibalizumab-uiyk be taken?

• Ibalizumab-uiyk is available as an injection which should be administered by a trained healthcare provider directly into the vein as a slow infusion for about 15 to 30 minutes.

• Ibalizumab-uiyk should only be diluted with 250 ml of 0.9% sodium chloride injection and should be administered within 4 hours if stored at room temperature (below 25°C) or within 24 hours if stored in a refrigerator (2°C to 8°C).

• The refrigerated solution should be allowed to come to room temperature for at least 30 minutes before administration.
• After the completion of each infusion of ibalizumab-uiyk, it is recommended to flush the medication out with 30 ml of 0.9% sodium chloride.
• Any unused portion or partially used vials of ibalizumab-uiyk should be discarded.

What are the warnings and precautions for Ibalizumab-uiyk?

• Patients should be monitored for the risk of immune reconstitution inflammatory syndrome which requires further evaluation and supportive treatment.

• Patients should regularly be monitored for laboratory abnormalities to detect variations of blood sugar level and various markers for blood disorders.

• Levels of bilirubin, creatinine, and uric acid levels should also be evaluated while the patient is on ibalizumab-uiyk treatment.
