Rehabilitation and shoulder surgery

The need for informed and skilful shoulder rehabilitation has built bridges between orthopaedic surgeons and therapists over the years. Societies such as the British Elbow and Shoulder Society (BESS) and The European Society for Shoulder and Elbow Rehabilitation (EUSSER) have become multi-disciplinary organisations, and there are regular "physiotherapy days" run in conjunction with many national, continental and international shoulder meetings. Some surgeons feel that the quality of post-operative rehab is as important to the final outcome as the choice of procedure, approach and devices employed.
Unfortunately, the many demands of surgical training often leave little time for formal training in rehabilitation techniques.
Rehab itself covers a broad swathe, as can be seen by current articles appearing in Shoulder and Elbow. In Upper extremity injuries in road traffic accidents, Wraighte et al have explored the range, severity and treatment cost of upper limb injuries to front-seat occupants in motor vehicle accidents, while in Ergonomic considerations for shoulder surgery in combat helicopter pilots, Piers et al have examined the range of motion requirements of combat helicopter pilots.
These studies help us to identify the likely injuries for certain patient groups, and also what their rehab needs will be for a subsequent return to function.
The patients themselves are important participants in the rehab process, and this is highlighted by Nassiri's et al paper Compliance with sling-wearing after rotator cuff repair and anterior shoulder stabilization who have explored the compliance of wearing a sling post-operatively. Their results are worth consideration by all surgeons and therapists.
Blacknall (Rehabilitation following reverse total shoulder replacement) provides us with a reasoned overview of the entire rehab process, and relates this to the available evidence. We look forward to more articles in this vein. Finally, we note that only one of the papers here is by therapists – a situation which we hope will change!

Breaking news: First new transplant drug in a decade granted FDA approval.
June 15, 2011 – The U.S. Food and Drug Administration has approved Nulojix (belatacept) to prevent acute rejection in adult patients who have had a kidney transplant.
This approval is of major significance for the field of transplantation and for thousands of kidney transplant recipients worldwide, as it marks the first new drug specifically indicated for transplantation to be approved in a decade. Following the FDA's approval there is now an opportunity and a challenge for health professionals involved in the care of transplant patients to determine how best to use this new anti-rejection strategy. There are still many questions about how this new drug (which requires a monthly infusion) will best be delivered to the patients who need it, and which patients are most likely to benefit from this new treatment paradigm...
