Hydroceles can be present at birth due to a patent processus vaginalis or develop at a later age due to an infection or inflammation of the testis.


Primary or Congenital Hydroceles

At about the eighth month the testicles descend from the developing abdominal cavity into the scrotum. A sac (processus vaginalis) accompanies each testicle, allowing fluid from the peritoneal cavity to reach the scrotum and surround the testis. By the time the baby is born or within a few months afterwards the sac closes.

In some cases the sac closes but the fluid remains and the condition is known as a Noncommunicating Hydrocele. Because the sac is closed, fluid cannot flow back into the abdomen. Usually the fluid gets absorbed within a year.

In some cases, however, the sac remains open and the fluid in the sac communicates with that present in the abdomen and it is known as Communicating Hydrocele.

Adult Onset or Secondary Hydroceles

In older men, any process that acts to stimulate increased production of watery fluid by the tunica or decrease the absorption of this fluid by the scrotal lymphatics or venous system will result in the formation of a hydrocele.

Increased production of fluid could be due to:

Inflammation of the testis (orchitis) or epididymis (epididymitis) caused by tuberculosis and by tropical infections such as filariasis.

Testicular torsion (rotation of the testis) may cause a reactive hydrocele in 20% of cases.

Tumors of the testis, especially germ cell tumors or tumors of the testicular adnexa may cause hydrocele.

Decreased resorption of fluid could be due to:

Surgery in the inguinal region or a renal transplantation can affect the lymphatics or venous system causing decreased absorption.

Radiation therapy is associated with cases of hydrocele.

Peritoneal dialysis and ventriculoperitoneal shunts.

My boyfriend has a softball sized hyrdocele. He has had difficulty keeping a job due to discomfort and sometimes pain from the weight of it. He has even had to bring a doctor's release to say it is okay on multiple occasions. It is so large that is tremendously obvious no matter what he wears. Because of the inability to keep a job, he has no insurance. He has been to the ER with the pain, but they say it is not an emergent condition. How large does it have to become before it is emergent and is there truly no complication to having it? I mean, if someone kicked him there, would it bust and if so, then what??? I am being serious.

my husband has to have hydrocoele surgery but is worried about the side effects is there any?will evrything go back to normal including sex life please let me no thankx

don't get worried, i have posted my condition as male24. He will be able to resume to all normal activities within 3 to 4 weeks at most. My testicle now gets significantly reduced still not to normal size but close to it and the condition is becoming better day by day.

@maximus.maxi of course there nothing to worry, just keep in touch with your doc. in my case i am recovered just there is a lill bit swelling left.

just dont forget to wear good scrotal support,nd apply icepacks to the area, rest will be fine.

Rt. testicle hydrocle without any complication Procedure followed: eversion of sac [jaboulay's procedure] had my hydrocelectomy on 29 march, the surgery was done within about half an hour and after that they placed a very big fat supporter on the operated area. for two hours i didnt felt any pain due to the affect of anesthesia. After that i had started feeling pain which was gradually increasing but it wasnt that much unbearable as they gave me some painkiller injection. The same day on evening i had another painkiller injection the pain is bearable. I urinated first time after 3 hours or 4 hours after the surgery and that was very tricky as the put that big supporters and due to the mild pain on giving the pressure to the bladder but managed it somehow for that day. Doctor adviced me to take more and more water and full bed rest for two days. on second day i felt much better to reach the toilet, supporters were changed at morning. on day 3: supporters removed,dressing was done on the stiched area, waliking becomes much easier, very less pain, scrotum swelling is as much as it was before surgery i.e. lika a normal size potato, on day 4: again dressing, i recognized my scrotum size same as on day 3 and the testicles feel bigger and harder, doctor told its common. day 5: sutures removed, i didnt confirm ny change in the size. after day 6: came home, yes they took me for that much time(it was army hosp. and everything is free). i continuously using 2 pillows between my legs when laying sideways. testicles are still harder and big but not the same as on day 3. today: applied some ice on the testicle, felt really great, slept for 3 hours in noon time and after that i found some more reduction in swelling almost 5%, but testicles are still hard like some delicate rubber cork. after coming home no medication required, just cold water bath and rest as much as possible, slight walk. very mild pain is there. one more thing on day 3 and 4 i had very hard stool and applying presure gave me very pain yet i managed and didnt tell anyone (crazy decision) on 4 th day night and till 8th day i took stool softener and that really worked. Now i stopped taking stool softener and any medicines and everything going fine except the swelling which i sometime doubt (since its actually feeling like big testicle and that also hard). will post update later

Boyfriend had surgery 2.5 weeks ago, is shocked that this surgery has taken so much out of him and how much pain he has endured. He said no other surgeries, including his Vasecetomy was difficult. He had taken two bottles of Vicadin, and did not even think he would need the pain meds at first. He is slowly, but surely getting better now each day. We finally had sex after the two and a have weeks, but he is having some discomfort afterwords. He is 44 and is extremely active and healthy. He is glad it is over and never wants to deal with a surgery like this again!

So I am in my third week after having one removed in my right side. It still has yet to shrink, I think there is liquid in it still, is that normal? Will it shrink? I was told to wear a jock strap but I have been wearing beefs so far, could it just be drained? Please don't just tell to listen to the doc ect, mine was in a connection to a hernia on the right side. It was a communicating cele, and if I wear the strap to wear it is effective it is to tight around my waist area an it begins to feel very uncomfortable [hernia was near there].

my left testical is still swollen and hard after 3 months from cyct and hydrocele surgery, surgeon doesn't seem concerned, there is little or no pain involved

i had A HYDROCELE REMOVAL OPERATION ON MY LEFT TESTICLE, IT WAS THREE TIMES AS BIG AS NORMAL, IT HAS BEEN ONE WEEK SINCE I MAD THE SURGERY,I DO NOT FEEL PAIN,BUT I DO FEEL UNCOMFORTABLE AND SWELLING AND STIFFNESS ON THE SCROTUM,MY FEARS WERE ABOUT THE SIZE DIFFERENCE. IT IS TRUE IT ONLY HAS BEEN ONE WEAK SINCE THE OPERATION,BUT I STILL HAVE WORRIES ABOUT THE SIZE DIFFERENCE.IS IT GOING TO BE THE SAME, OR THERE IS GOING TO BE A BIG DIFFERENCE. MOREOVER, ON THE RIGHT SIDE,THE SKIN HAVE BECOME hanging granted.WILL IT BECOME BACK NORMAL AFTER FEW MONTHS. what should i do, how can this skin dissapear or constrict? THANK U.. Im a 19yr old male that just had a hydrocele ectomy. My Dr. placed a drain tube in the incision that will be removed friday. Other than slight pain and discomfort the recovery isnt awful as of so far. My only real issue is I cant eat or drink so far. Keeping liquids down is nearly impossible as of right now. Any tips on how to get over the nausea? Also when a hydrocele ruptures is there alot of pain involved? I had increase of pain and swelling a few weeks back, im just curious if that was caused by the rupture.

The Doctor has recommended the removal of the one testicle explaining that there is a lot LESS post op pain, no swelling, less risk of infection vs performing the Hydrocele surgery-Removal does not impact sex, or prevent sex however concerned because the most common procedure appears to be the Jaboulay procedure which sounds like lots of pain, bruising and swelling-Why go through that if recovery is less for the removal of the testicle-Age 68 great health! Any one out there getting this recommendation for hydrocele?

Astonished the doctor wishes to remove a perfectly healthy testicle, I had Lords with Jaboulay and as i stated in a previous post the post op pain isn't really significant and is eased with painkillers and a warm bath. Yes there is some swelling but after three days i was back at work feeling fine..I think we are guilty on these discussions of painting a more severe picture of the minor op than it really is...have the op but keep the testicle would be my advice...all the best

I am to undergoe a 'LORD'S PROCEDURE',for a left side Hydrocele in October. It appears to be connected with the Hernia, which was repaired for the second time a few months ago. I wonder if it is worth all the discomfort that the various comments mention. It not life threatening, only mildly uncomfortable and mainly a nuisamce. I am 74, but in good health.

I had a Lord's Procedure last Thursday. Amazingly, NO PAIN just slight discomfort from bruising and suitures. Thoroughly Recomend It!!! EVERYTHING! already back to normal as well!!!

Thank you for posting! My dad is 73 and we're going tomorrow to get his hydrocele looked at. He had 2 hernia surgeries back in July and now this. He's feeling pretty low, but I keep trying to find stories from people his age who have had it done and feel better. :]

to which doctor u have shown, is really u r scrotal in normal position. actually i am suffering from 1 and half year, there is no pain but size matters.
