Acute myocardial infarction

Hypoxemia with respiratory distress

Hypotension and shock, regardless of etiology.

Massive upper GI bleeding with hypotension where emergency surgery is clearly appropriate.

Uncontrolled hypertension.

Un co-operative patient.
