Posted on: 05/20/2012 dear sir, i click photo before 6 months but i don't receive photo id card.i checked about my card but don't reply in esic office. our agent talk to me,your esic offices system is very careless and they haven't knowledge, how to talk with our ip[employee], so please give them training(asarwa office and naroda g.i.d.c office
so please help me in this matter

My IP No- 6921469385 Name- Gulshan Kr Dogra DOB- 09.04.1978. On 09/10/2011 I clicked family photo at ESIC HOSPITAL NH-3 Faridabad.But Still i couldnot get my Smart Card. Two times i confirm it in branch but not get any satisfactory answer. Plz Short out my issue and Provide me my smart card as soon as possible. Thanks Gulshan Kr Dogra 09311118188

Posted on: 01/16/2012 i have not received my smart card i have done all formalities at ESI office and hospitals I am waiting for card for more than one year. hereby i would like to request you do needful as soon as possible because I will not get my medicine after some time.I hope you will help me because no one help me for the same. what is the problem for issue of PIC to employee. what is the process and why it's take too much time to provide so long time for PIC. who is the responsible for that please let me know and kindly do needful as soon as possible. Thanking you.

Posted on: 03/26/2012 you can get your Temporary ESIC card from your Employer and paste Your postcard size photograph with all dependents [family members,that temporary card will attested by your Employer with that photo and you can take it to your designated Dispensary (stated on card]and your your card will be prepared and send to u in 15 days |
