Dependence Certificate
Every Card Holder must certify that the parents/dependents whose names have been included in the token card, reside with him/her in order to be eligible for CGHS facilities. Such a certificate must be furnished at the time of issue of Token Card. The Dependence Certificate must be renewed in January yearly.
In case, the concerned Government Servant is unable to furnish Dependance Certificate, such an employee’s parents shall be treated as non-entitled persons under the ambit of CGHS Act. The certificate must incorporate a statement or a clause that the income from all sources accounted together does not exceed Rs. 3500 + DA (Daily Allowance) per month

For any pensioner/family pensioner entitled for benefits under CGHS Act, the rate of contribution will be with reference to the grade pay that he/she would have drawn in the post held by him/her (at the time of his/her retirement / death), had he/she continued in service till now, but for his/her retirement/death.
