An emotional video from a thirty year old mother of two children is now on YouTube, and it poignantly reveals the story of her battle with a severe heart disease with a parting message to her loving family.

Her two children, presently too young to understand, may be in a position to appreciate her message in the video a few years later.

Jennifer Johnson was informed of her chronic heart condition after her marriage, while she was pregnant with the first child. She had to undergo a surgery to fit a pacemaker.

In October this year, a wire of the pacemaker needed to be fit as it had become loose and symptoms of her condition came back dangerously. She was told that her days were numbered and will perhaps live just for a few months. The family could also not bear the mounting medical costs. In an emotional moment she decided to make a video of the story of her life and her medical condition, in a bid to raise funds to take care of the children as well as create awareness of heart disease.

Jennifer died on December 17th, but before she died, she used flash cards to tell her two small children about the story of her life.

Her aggrieved husband remembers her optimistic spirit and has now released this footage to improve awareness about heart disease and raise some funds to help care for their children.

Jennifer Kordecki Johnson is my niece and this is her heart story. A lot of people have turned her story into a debate over national healthcare, but I want to bring this story back to what was important to her... her heartbeats. Jen was a woman who lived fully with her heart as her barometer... the tragedy is that she also had very, very sick heart. Far sicker than any of us knew. Her HOCM was a genetic condition that went undiagnosed until two years ago, but she was determined to live to see her babies grow up. A septal myectomy was the only treatment option that could provide her an opportunity to do just that. While open heart surgery is very serious and comes with great always think this kind of thing will not happen to your family...but it did. I held Jennifer in my arms as she went from this world to the next, and while we could not save her, we can help her two beautiful children, whom she called her heartbeats.
