MINNEAPOLIS, July 27 Premier Visiting Nurse, part of Ciena Healthcare Management, Inc., announced that it has entered into a business relationship with Cardiocom(R) to provide advanced home telehealth technology to patients with chronic illness. Cardiocom, the Experts in Telehealth(SM), is a world class developer, manufacturer and clinical service provider of award-winning telehealth solutions.

Cardiocom's telehealth system provides an interactive opportunity to proactively manage patient care. Every day, Premier Visiting Nurse patients use the Cardiocom Commander Telehealth System to communicate with a Premier home health nurse. The Commander Home Telemonitoring System guides the patient through an interactive Health Check and collection of vital signs including: weight, blood pressure, oxygen saturation and heart rate. This information is immediately transmitted via a standard phone line to the patient's caregiver for review.

"Cardiocom provides an interactive means to proactively manage our chronically ill patients, thus reducing hospitalizations and improving overall patient satisfaction," says Patricia Bartlett, RN, Director of Clinical Operations, Premier Visiting Nurse. "By transmitting objective, real-time health data every day, the patient is connected to our medical team so that their care plan can be adjusted as needed. This system uses today's most advanced home care technology and monitoring services. With Cardiocom in a home care setting we have the peace of mind, that our patients are being monitored on a regular basis, updating the physicians and thus providing overall quality care. This system not only helps the home health agency, but also will help reduce healthcare costs to the Medicare program," adds Talha Mirza, Administrator, Premier Visiting Nurse .
