This conference will examine innovative care delivery and coordination models, lessons learned from providers who have expanded services and strategies for making decisions to expand care. Presented in cooperation with the Hospice & Palliative Care Federation of Massachusetts, National Association of Social Workers, National PACE Association, and the Center to Advance Palliative Care.
