NASSAU, N.Y. and QUEENS COUNTIES, N.Y., Jan. 25, 2018 /PRNewswire/ -- The Nassau Queens Performing Provider System (NQP)—a Performing Provider System of the New York State Department of Health's Delivery System Reform Incentive Payment (DSRIP) program—is providing $1.8 million to 33 community based organizations (CBOs) in Nassau and Queens for health improvement initiatives. Approximately $1.1 million will be provided to 23 of those agencies through NQP's CBO Innovation Fund—a fund created to provide CBOs with the financial resources to assist them in developing health-related programs.
"We recognize how grass root organizations who provide services such as social services, food, housing, community education, and employment placement can help shape and improve the overall health of a community," says Bob Hettenbach, NQP's executive director. "They may not be viewed as health care organizations, but they have relationships with individuals in their community that could be helpful with engaging people into the health care discussion. In the initial phase of our strategy, we will utilize these relationships to provide education about illnesses, and teach the community how to better use the health care system."
While the individual focuses of the 33 CBOs may vary, they share a common theme of providing basic social services within NQP's hotspots—College Point/North Flushing; Flushing; Fresh Meadows; Glen Oaks/Bellerose; Briarwood; Jamaica/St. Albans; Jamaica/Hollis; Inwood; East and West Rockaways; Arverne/Edgemere Far Rockaway; Wave Crest Far Rockaway; Glen Cove; Central Westbury; North Hempstead; Central Hempstead/Uniondale; Roosevelt; and North Freeport.
As NQP's relationships with CBOs mature, selected organizations will receive additional funding for increased staff.
"We see great potential in the use of Community Health Workers (CHWs), to assist individuals with chronic health conditions in navigating the complex health care system," explains John Javis, NQP's Director of Behavioral Health. "CHWs tend to live in the neighborhoods in which they work, and very often have successfully managed their own health conditions such as diabetes. These "peer" elements enable the CHW's to motivate those they serve to improve their own health outcomes."
The organizations receiving funding include Rockaway Waterfront Alliance; Gateway Youth Outreach; Venture House; Long Island Council on Alcoholism and Drug Dependence (LICAAD); Rockaway Youth Task Force; Beacon House (UVBH); Make the Road New York; New York Association of Psychiatric Rehabilitation Services, Inc. (NYAPRS); Belmont Chaplaincy; Long Island Coalition for the Homeless (LICH); Mental Health Association in New York State, Inc.
