CareManager modules are now available for seven disease states, includingdiabetes, stroke prevention, coronary heart disease, cancer screening, tobaccocessation, heart failure, and osteoporosis. The 3.3 release also includesupdates to existing modules, providing the most up-to-date industryestablished guidelines for the evidence-based care of patients with chronicconditions.

Providence Medical Group (PMG), an employed group of 130 primary carephysicians in 12 practices in and around Portland, Oregon, has been usingCareManager since 2001 and seen significant improvements in the outcomes oftheir patients and the efficiency and functionality of its practice. By theyear 2000 PMG groups had virtually eliminated paper records; however, theysoon discovered their outcomes for diabetes were no better than the nationalaverage. Following the implementation of CareManager, Providence demonstrateda 58 percent increase in the number of diabetic patients who achieved controlof their cholesterol and blood pressure levels. These improvementssignificantly reduced the risk of heart disease across a diabetic patientpopulation of over 6,000. Providence also documented a 250 percent increase infoot screenings, helping to stem the rate of later amputations. While usingCareManager to ensure patients received the care they needed when they neededit, Providence generated more than $4,000 of additional service revenue perphysician in the first year of use, without hiring any additional staff.
