Care Improvement Plus Recognized with Metropolitan Partnership in Aging Award

ATLANTA, Nov. 19 Care Improvement Plus today announced that it has been recognized with an "Emerging Health Model Partner Award" from the Atlanta Metropolitan Partnership in Aging. The award was given to Care Improvement Plus for developing a series of health education focused events for senior residents in the Atlanta metro area in partnership with the Atlanta Area Agency on Aging Wellness Program and the University of Georgia Cooperative Extension Service.

"This alliance is a unique model in that it engages chronically ill seniors within their own communities and provides them with the tools to live a longer, healthier, happier life," said Jennie Deese, Southeast regional director of community outreach for Care Improvement Plus. "We are thrilled to have been recognized, and feel that this is a wonderful example of how finding a common thread fosters partnerships and successful collaborations."

During the past year, the three-organization alliance held more than 20 events throughout the 10-county Atlanta region which were attended by more than 1700 area seniors. Events were conducted in partnership with additional community organizations, providing valuable health education including "how to" strategies that encourage area seniors to take an active role in their health and the prevention of chronic disease. As Georgia ranks 9th in the country for prevalence of diabetes(1), the alliance felt strongly that the focus of the events needed to be on healthy living strategies to prevent chronic illnesses such as diabetes and/or their complications.



"The Atlanta Regional Commission is pleased to recognize Care Improvement Plus and the services they have offered over the past year to improve the health of older adults," said Cathie Berger, director of the Atlanta Area Agency on Aging. "This award highlights the commitment of Care Improvement Plus and also the tremendous value of what can be achieved through partnerships that bring together agencies from the public and private sectors."



Ten more events are scheduled through November and many more are in the planning stages. Recently, Care Improvement Plus announced that it has partnered with the Area Agency on Aging (ARC) to sponsor the DeKalb and Fulton County SPARC coalition events, which provide immunizations and other valuable health services to area seniors.
