1. When to use antipyretics ?
Purpose of antipyretics in fever is to prevent fever from rising and to offer comfort to the patient. Hence it is indicated only in moderately high fever and in a child who is irritable and uncomfortable due to fever.
There is no role for round the clock antipyretics, use only as per need.
Prompt usage is recommended in children with febrile seizures, especially during the first 24-48 hours.
2. What antipyretic to use ?
Paracetamol should be the antipyretic of choice in routine control of fever.
Fixed dose combinations of two antipyretics is not recommended.
Combination of antipyretics with metoclopramide or methionine is not recommended.
3. What are the non-pharmacological methods of control of fever?
Switching on the fan or the air-conditioner.
Remove tight or excessive clothing or cover.
Tepid sponging with luke warm water (Use of cold water or ice promotes vasoconstriction and does not allow the heat to dissipate, hence avoided).
Paracetamol, "the antipy retic" in pediatric fever.
