Generic Name : Clomethiazole.
Trade Name(s): 
International- Heminevrin capsule.
Why it is prescribed (Indications) : This medication is a hypnotic and sedative, prescribed for treating and preventing symptoms of acute alcohol withdrawal, sleeplessness, restlessness, and agitation (excitement).

When it is not to be taken (Contraindications): 

Acute lung insufficiency.

When it is to be taken : Oral- Insomnia- 1 or 2 capsule (192 or 384 mg of the base or the equiv)/day at bedtime.
Alcohol withdrawal syndrome- Per capsule contains clomethiazole base 192 mg: 9-12 cap/day in divided doses on 1st day, reduce dose gradually over the next 5 days. Not to be used for >9 days.
Sedation- 1 capsule (192 mg of base) 3 times/day.
IV- Status epilepticus as 0.8% solution: Initial: 5-15 ml/min, up to 40-100 ml. Usual maintenance: 0.5-1 ml/min. Alcohol withdrawal syndrome as 0.8% solution: Initial: 3-7.5 ml/min until shallow sleep is induced, then reduce to 0.5-1 ml/min to maintain sleep and spontaneous breathing.
Pre-eclamptic toxaemia as 0.8% solution: Initial: 0.5-0.75 ml/min during labor. Maintain at 0.5 ml/min for 12 hour post-delivery.
How it should be taken : It comes as a capsule to take by mouth, at bedtime. It is also available as an injection, to be administered by a physician.
Special Instruction : 
* Caution should be exercised in patients with history of chronic lung insufficiency, heart, respiratory disease, sleep disorder, liver or kidney impairment, drug abuse, marked personality disorder, elderly, children, during pregnancy, and breast feeding.
* Excessive sedation and drowsiness may occur.
* Maintain integrity in the airway region (duct that is responsible for ventilation) during parenteral administration in order to avoid respiratory complications.
* Avoid prolonged usage.
* Keep regular appointment with the doctor.
Side Effects : 
Most Common- Headache, nasal congestion and sore eyes.
Eye and ENT- Irritation in nose and eyes.
Central Nervous System - Excitement, confusion and dependence.
Skin - Rashes, hives and allergic reactions.
Metabolic- Alterations in the level of the liver enzymes.
Potentially Fatal- Respiratory depression.
Other Precautions : 
*It may cause dizziness, lightheadedness, do not drive a car or operate machinery while taking this medication.
* Avoid abrupt withdrawal of the medication.
Storage Conditions : 
Intravenous Store it in refrigerator (2-8°C). Keep away from children and moisture.
